Default replacing the default icons.dll

I have a C3PO that adds a toolbar button and works perfectly when I click on it. This is done with the using the default icons.dll file.

However, when I try to create a new file with Visual Studio 2008, it doesn't show up. I have extracted the files from the default icons.dll and used those same exact icons and bitmap to try and create something new. Everytime I do that, it never shows up in the toolbar. This is a stupid question but can somebody give me a starting point to creating either a new dll or exe that can be used with the C3PO API in VS2008? I read somewhere that it can be in an exe or dll file and I've tried just about every kind of those file types in VS2008 and nothing works.
