ModifyItemRequest seems to do nothing on existing Documents

Sorry Preston, I did not get what you meant by only specifying the property that needs to be changed. Now I can see what you implied was that I should declare a new document and set only the property that I need to modify since the document id is passed to the ModifyItemRequest.
I've now managed to change the subject, and then added a custom field on the document.

I don't understand.
In the modifyItemRequest that I see in the trace,
the whole item is passed in the <update> element,
not just the <subject> or <customs>/<custom> element
as I would expect.
