Unlikely to be a database issue. That's a communication or routing issue
in the other direction, e.g between the final receipient of the message
inside GW (another PO, or GWIA), and the problem PO, e.g the messages
that are supposed to travel back to the sending PO telling it about a
status change never make it there.
