Notes 4043 errors

Just performed an upgrade from IDM2.01 to IDM351 and our Notes drivers are experiencing problems. They either cannot open the ID file (see trace below) or throw up a 4043 error saying they cannot open the database (same symptoms also in the trace).

All passwords and security levels have been double checked so any advice would be appreciated. I have followed the steps outlined in TID 10092978 with no success.
