Nothing to do with your problem, but you should disable SMP in this server, there is no gain and possible pain for more than 1 CPU in NW 4.x . So remove these lines from startup.ncf and reboot:
LOAD MPS14.PSM
LOAD SMP.NLM
LOAD MPDRIVER.NLM ALL
