Disappearing Trustee Rights

There is a recurring problem, where files in a particular directory's sub
directories lose the rights that have been configured. We've found both of
the following occuring:
1) The trustees and the rights disappear
2) The trustees and rights do not disappear, however users with
permissions cannot access the files.

We've solved this by restoring the permissions from backup, but are
obviously interested in detecting the cause and putting a stop to it.

I have found no information in Novell Knowledgebases, so I'm hoping
somebody around here has seen something similar.
