I had the same problem and it came down to 3 issues....

1) The version of nwtape.cdm that ships with NW65sp8 leaves a tape drive
in a non-usable state when it is unloaded. You need to rename the file so
that it NEVER loads.

2) BackupExec 9.2x doesn't always update correctly. By checking versions
of the ndmpd.nlm I found out I was running a 4.0x version instead of the
one I had patched to. Once I manually copied the file over I could see that
I was getting a PCI interrupt error in logger instead of a volume dismount
followed by a hard abend.

3) The Adaptec 39320 PCI-X controller that I was using had worked fine in
a DL585 in a 100mHz slot, but failed in a 133 mHz slot (even though the card
is "rated" for this)., it would dismount the tape drive and kill the backup.
Replaced the card and haven't had any problems since.
