Importing Twice

I've created a new image...the same way I've created images in the
past.....but this time I have a small problem I can't seem to track
down. When I image a new WS with the image, it is being imported twice
on reboot. I wind up with "MLES-LAP-01" and MLES-LAP-011".

What would cause a WS to import twice?
