Adaptec AAA-131U2 Raid Controller with 5 18 GB Fujitsu drives
4 Volumes (SYS-10GB, Archive-20GB, ACCT-20GB, DATA-20GB)

Server hung on Thursday morning, no error message, could not down it and
had to do a cold boot. When it came back up it appeared to load okay
until it got to mounting the DATA volume, while reading the directories
it gave several beeps and gave the following error messages:

Error writing to the directory on PCL/SYS

Device [V316-A1-D0:0] ADAPTEC ARRAY01" deactivated by driver due to
device failure

Replaced the raid controller and got same result

Ran server with -ns and -na switches and edited the autoexec.ncf to stop
the mounting of volume DATA, rebooted and everything came up with no
problems.

Tried to manually mount the DATA volume and it crashed again with the
same errors - once it stops, the sys volume is also gone, so have to reboot.

Rebooted with -na and tried to run vrepair on the DATA volume, it runs
for about 2 minutes and then emits several beeps and returns message
saying vrepair was not completed. The system console screen has the
following error messages:

NWPA-004: CDM driver deactivated device [V316-A1-D0:0] ADAPTEC ARRAY01
due to device failure


SERVER-5.60-1534 [nmID=B0013]
Device [V316-A1-D0:0] ADAPTEC ARRAY01" deactivated by driver due to
device failure

Any thoughts?

What I have tried:
Replaced the Adaptec Raid controller and the driver files
Replaced the RAM on the motherboard (it will only accept 1GB, so could
not increase the amount of RAM)

In the clutching at straws category, I have run DS Repair, vRepair on
other volumes, and tried everything using only my left hand - nothing
has worked.

Any suggestions will be greatly appreciated
