I am having a recurring issue with an HSG80 SAN device dismounting, leaving the following error.

Severity = 0
NWPA-004: The CDM driver deactivated device [V597-A2-D3:C] DEC HSG80 rev:V86F due to a device failure.

3-08-2008 3:13:22 am: COMN-3.24-1092
Severity = 4 Locus = 3 Class = 0
NSS-3.00-5001: Pool NILNW09/RA2 is being deactivated.
An I/O error (20204(zio.c[2260])) at block 67185059(file block 212224)(ZID 3) has compromised pool integrity.

Device "[V597-A2-D3:C] DEC HSG80 rev:V86F" deactivated by driver due to device failure.

This issue has occured at a fairly close interval, every 7 days.

Another LUN connected through the same HBA has no issues, nor does any other server connected through the same fabric and san controller.

A little inexperienced on troubleshooting something like this, if someone could nudge me in the right direction.
