Notice that do-set-dest-attr-value is not the same as
do-add-dest-attr-value and without matching these two I'm not sure what
would happen. In your case it appears to actually get past this so
therefore it must (in my mind) be a typo.

Moving on to your real symptom, post a full trace of an event from the
LDAP source. Without this any number of things could cause the error
you described. Specifically this looks like a problem with a modify
instead of an add which was not what I expected to see at this point.
When we can see the full trace I think this will become much clearer.
