A couple of things to try....

First, go through the MAD driver documentation and be sure your user MAD
user has rights. I know, it's Administrator, but maybe it's possible to
change that... who knows.

Second, is the RL running on a DC? If so, clear out the Authentication
Context field entirely of the DNS name of winvmdc27.melb.devad since
that is redundant and can sometimes cause issues.

Third, you're on IDM 3.5.1 anyway so at least upgrade the Remote Loader
to 4.0.2 and use the latest shim. Is this a supported setup? No, but
it should be fine, and you're out of support anyway by being on 3.5.x.
Doing this will get you a newer shim and newer password sync filters
which may improve things in the long run.

My best guess would probably have been deleting the state file and the
DirXML-DriverStorage attribute; maybe be sure you did that on the right
server since it is a per-replica attribute and you may have deleted (or
tried to delete) from an invalid machine.
